Patients are responsible for providing complete and accurate medical history, including all current and past diagnoses, medications, and mental health conditions.

Failure to disclose relevant health information, including psychiatric conditions. may impact the safety and appropriateness of treatment and may result in modification or discontinuation of care.

Our providers rely on accurate information to make safe clinical decisions.

Restore Medical provides care within defined clinical protocols designed to ensure patient safety and consistency.

We reserve the right to decline, modify, or discontinue treatment at any time if:

• A patient’s condition falls outside our scope of practice
• Treatment is not tolerated safely
• Required monitoring (labs, follow-ups) is not completed
• There are concerns regarding safety, compliance, or behavior

In such cases, patients may be referred to a specialist or higher level of care.

Certain medical and psychiatric conditions may affect the safety and appropriateness of hormone therapy.

Patients with complex or unstable mental health conditions may require coordination with a primary care provider, psychiatrist, or specialist before or during treatment.

Our clinic may defer or discontinue therapy if we determine that care would be better managed in a specialized setting.

We work with a select group of licensed pharmacies that meet our quality and regulatory standards.

We are not able to:

• Use outside pharmacies
• Accept patient-supplied medications
• Customize compounded formulations beyond our established protocols

This ensures consistency, safety, and compliance with state regulations.

All services provided by Restore Medical are non-refundable.

This includes consultations, lab reviews, treatment planning, and medications once processed or dispensed.

By initiating treatment, patients acknowledge and agree to this policy.

Testosterone therapy is a regulated treatment that requires ongoing monitoring.

Patients must complete required lab work and follow-up appointments within designated timeframes in order to continue receiving prescriptions.

Failure to complete required monitoring may result in delays or discontinuation of treatment.

We are committed to maintaining a safe and respectful environment for our staff and patients.

Threatening, abusive, or inappropriate behavior, whether in person, by phone, or electronically, will result in immediate termination of care.

If there are concerns regarding patient or staff safety, appropriate authorities may be contacted.
